There's a hostage crisis going on in downtown Pittsburgh right now. (I'm not directly affected, I'm at the other end of the downtown area)

Alleged 22 year old is holed up in an office down near the point, took a man hostage at about 0800 EDT. Police have been on the scene since.

First TV station on the scene was KDKA. Considering that the crisis is going on in Gateway Center Building #3, and the studio is located next door in Gateway Center #1, that's almost a no-brainer.

I go down at 12:30 to eat my lunch. The usual ladies are there, quite irritated that their soap opera has been pre-empted by the news coverage.

And then they start laughing.

Seems that there was a typo in the "bulletin" banner on the bottom of the screen.

It was informing us that we were watching "Breaking Mews" (and that stayed on-screen for over 20 minutes)

Easy enough to make that mistake. I was a Chyron operator as part of my broadcast duties back in the day. The station had a policy that one person typed in the data, and all the data had to be reviewed by a second broadcast staff member. With cutbacks in staff and more and more automation making people redundant, we can only expect that we will see more screw ups like that as fewer and fewer people work in station control rooms, especially when the pay is abysmal.
