The instructor should lose his certification, for sure. Safety is his first and foremost responsibility, particularly around a group of folks who may be total novices.

I still like the incident where the dog shot his owner in the ass with a shotgun. It just happened a month or so ago, but I'll be damned if I can find a link to it.
